ABSTRACT

Resumo: Introdução: Apesar de o ensino médico brasileiro ser pautado pelas Diretrizes Curriculares Nacionais (DCN), a atenção primária à saúde (APS), como cenário de prática, ainda é negligenciada nos currículos de muitas escolas. Objetivo: Neste estudo, buscamos compreender o uso da série Unidade básica como um recurso educacional de reflexão com o foco na APS. Método: Trata-se de um estudo qualitativo baseado na análise de entrevistas realizadas com docentes de diversas escolas médicas que utilizam a série em variadas disciplinas. As entrevistas foram gravadas, transcritas e analisadas segundo a fenomenologia hermenêutica de Ricoeur, gerando os resultados do estudo. Resultado: Realizaram-se 18 entrevistas com docentes da graduação em Medicina e preceptores e tutores de programas de Residência em Medicina de Família e Comunidade. Os docentes mencionaram que utilizam os episódios da série como incentivo para o debate dos princípios teóricos e atributos da APS de maneira mais interativa e interessante para os alunos. Os entrevistados relataram uma ampla aceitação do material pelos discentes, aumentando a participação em sala de aula, inclusive em períodos de ensino a distância. A maior crítica feita ao material foi a falta de representatividade da equipe multiprofissional da APS na série. Conclusão: A série pode apresentar grande potencial para a reflexão sobre a APS como estratégia de atenção, além de aproximar, de maneira lúdica, os alunos do tema e aumentar o interesse na prática médica generalista.


Abstract: Introduction: Medical education in Brazil is guided by the National Curriculum Guidelines (DCNs), however, primary health care (PHC) remains neglected in medical curricula. In this study, we sought to understand the use of the Unidade Básica TV series as an innovative educational resource in PHC. Methods: This is a qualitative study based on the analysis of interviews carried out with professors from different medical schools who use the series in different disciplines. The interviews were recorded, transcribed and analyzed according to Ricoeur's hermeneutic phenomenology, thus providing the results of the study. Results: Eighteen interviews were conducted with professors of undergraduate Medicine and preceptors and tutors of Residency programs in Family and Community Medicine. Lecturers reported using the episodes of the series as an incentive to debate, utilizing a script to address the theoretical principles and PHC attributes in a more interactive and interesting way for students. Interviewees report wide acceptance of the material by students, increasing classroom participation, including during periods of distance learning. The biggest criticism made of the material was the lack of representation of the cross-functional PHC team. Conclusion: The series has great potential for reflection on PHC as a care strategy. It can also bring students closer to the topic in a playful way and increase interest in general medical practice.

RESUMEN La política nacional de salud mental de la niñez y la adolescencia aún está mal estructurada en Brasil. En términos generales, se trata de una copia de su equivalente para adultos, lo que dificulta su vinculación con la red. El objetivo de esta investigación fue analizar y caracterizar las acciones desarrolladas en un Centro de Atención Psicosocial Infanto-Juvenil con camas, que atiende las 24 horas del día en la ciudad de Curitiba, Brasil. Se trata de un estudio descriptivo, de análisis de historias clínicas y datos de productividad del año 2016, que fueron provistos por el Departamento de Informática del Sistema Único de Salud (DATASUS). Los resultados muestran que el servicio realizó 29.449 procedimientos, atendió a 112 niñas, niños y adolescentes en la recepción de 24 horas. Las historias clínicas indican el amplio uso del servicio por parte de la red. El 52% de la demanda provino de la atención primaria, pero la otra mitad de una amplia variedad de servicios. Esa variedad de acciones sugiere una clínica más flexible, que reemplaza el hospital psiquiátrico clásico por los principios de la clínica ampliada. Además, la existencia de camas facilitó la administración de la crisis de forma más vinculada al territorio


ABSTRACT The National Policy on Mental Health for Children and Adolescents is still poorly structured in Brazil. In general, it replicates the policy aimed at adults, which creates problems for its articulation with networks. The objective of this study was to analyze and describe the actions carried out at a 24-hour Child and Adolescent Psychosocial Care Center (CAPSi) with beds in the city of Curitiba, Brazil. This is a descriptive study based on the analysis of medical record data and productivity data between 2016-2017, provided by the Department of Informatics at the Unified Health System (DATASUS). Results show that the center performed 29,449 procedures and attended to 112 children and adolescents through its 24-hour service. Medical records indicate extensive use of the service by the network. Primary care accounted for 52% of demand, while the other half could be attributed to a wide range of services. The variety of actions carried out suggests a high degree of flexibility at the clinic, replacing the classic model of the psychiatric hospital with the principles of the expanded clinic. Furthermore, the existence of beds made it easier to manage crises in a manner more closely linked to the community.
